42-year-old man with persistent pain below his left ribs, several days of night sweats and fever, and unintentional weight loss of 15 pounds for 2 months. Physical examination revealed multiple prominent lymph nodes measuring 2-3 centimeters in the left axilla. CT abdomen revealed paraaortic and paracaval adenopathy and lymph adenopathy in the small bowel mesentery. Patient received 8 cycles of R-CHOP chemotherapy with persistent disease and developed acute renal insufficiency, weight loss, pain and disease progression and died 15 months after diagnosis.
